About Us

Person Centered Home Health Care LLC provides home and community-based services to individuals struggling with homelessness, physical and developmental disabilities, and mental health illnesses.


Our goal is to provide passionate care and services that align with people's lifestyles. 


Our focus is to form incredible relationships with our clients while providing services. We want to invest in developing interpersonal relationships with our clients by providing person-centered care. 

Our Services


Adult Companion Services


Non-medical care, supervision and socialization provided to a person age 18 or older. Adult companion services cover services that help a person work toward a therapeutic or community integration goal in their support plan.

The adult companion will:

  • Attend a movie with the person to practice coping skills to manage their social anxiety and go with the person to a community event to reduce their social isolation.
  • Play a board game with the person to enhance their fine motor skills
    Provide verbal instructions or cues to the person to help them complete tasks.
  • Assist or supervise the person with tasks such as laundry, light housekeeping, meal preparation and shopping.

Homemaker; cleaning and home management


Services that help a person manage general cleaning and household activities. There are three homemaker services:

• Homemaker/cleaning
• Homemaker/home management
• Homemaker/assistance with activities of daily living (ADLs).

Respite Care; in home/out of home


  • Short-term care services provided to a person when the person’s primary caregiver is absent or needs relief.
    In-home respite: Respite provided in the person’s home or place of residence.

    Out-of-home respite: Respite provided outside of the person’s home or place of residence.

Individual community living supports (ICLS)


Bundled service that includes six service categories. ICLS services offer assistance and support for older adults who need reminders, cues, intermittent/moderate supervision or physical assistance to remain in their own homes.
Service focuses on:
• Active cognitive support
• Adaptive support service
• Activities of daily living (ADLs) support
• Household management assistance
• Health, safety and wellness
• Community living engagement

Individualized Home supports with training


  • Cueing, skill maintenance, guidance, instruction, assistance with activities of daily living, assistance with coordination of community living activities or direct supervision.
    Skill-building and instructional services to acquire, retain and improve the person’s experience living in the community.
